Documentation Requirements- Gov’t issued ID of candidate resident and guardian, Signed Contract for Services, Signed Advance Directives Form, Transition Care Form (if coming from different facility), Senior ID/PWD ID.

The Club Retirement Villa is situated 24km away from Taal Lake/Volcano and is NOT part of the CRITICAL Area even if the volcano erupts. No ashfall was experience during the 2019 eruption.
WHY THE CLUB RETIREMENT VILLA?
Our difference with the other care homes, is that all each care packages is highly individualized. We also monitor our residents in a way that continuous quality life care can be provided in the best possible manner. Our multidisciplinary model of care aims to support independence, dignity and freedom of choice for the resident. The Club is more than a care home, it is a place where residents are treated as an extended family.
